St. Mary’s Academy Spring Fling Dance

We, the undersigned parent/guardian, agree for the student to go to the Spring Fling Dance at Kennedy Catholic Preparatory School. My son/daughter and I understand and agree to the following conditions:

• The student must follow the rules and regulations as outlined in the St. Mary's Academy Code of Conduct, the Attire Guidelines and the Arrival/Departure Policy.
• Ticket must be purchased in full by Wednesday April 29, 2026 at 11:59 pm. Students and parents understand that there is LIMITED space and tickets are based on a first come, first serve basis.  
• It is the Student and the Parent/Guardian’s responsibility to read and fully comply with the rules and code of conduct required by St. Mary's Academy and the Archdiocese of New York.
• All school rules and regulations are to be followed and enforced.
• I understand that failure to do so may result in my child’s expulsion from the event.
• My child is subject to be searched if deemed necessary by St. Mary's Academy Administration and I give permission if this were to take place.
• Once paid, there will be absolutely no refunds, and no exceptions for any reason.
• I authorize the school to obtain, through a physician of its choice, any emergency medical care necessary for the student in the course of such activities or such travel.
